Goal:
To create a personal behavior change plan utilizing the theoretical framework of the Stages of Change Model (Proschaska and DiClemente, 1983).
Objectives:
1. Students will identify a personal health and wellness behavior area to change
2. Students will develop a written wellness behavior change plan based on the SOC.
3. Students will demonstrate a strategy for implementation for over 6 months.
Instructions:
Choose one wellness behavior to change. It need be ‘urgent’; if you never change it, life will be less effective. Choose a behavior specifically related to your personal current health status; however, make sure it is precise. For example, if you want to improve ‘physical wellness,’ choose a small detail (body fat percentage, cardiovascular health) to improve. The plan is for a six-month period. Your project will have 3 major sections, outlined below.
· The entire project should be in Times New Roman 12-pt font and single spaced.
Include:
o a cover sheet stating the name of your project, name, and date
o A bibliography in APA format

Section I (25 points)
A 500 word explanation of the behavior you would like to change.
Why do you choose this and how has it had an impact on you?
If you do not change, how will it negatively influence your future? Identify and describe one ‘defining moment’ (specific event or circumstance) which convinced you to change no matter how difficult it will be.
Section II (25 points)
A one page literature review that analyzes your behavior in terms of gender, ethnicity, socioeconomic status, age, region, or culture. You will find three sources that support your analysis from valid and reliable journals (LSU databases). The literature review will be set up in this format:
Paragraph I – Introduce your behavior change topic and your analysis
Paragraph II – summarize source
Paragraph III – summarize source
Paragraph IV – summarize source
Paragraph V – conclusion
Section III (50 points)
A two-page integration of your behavior change with the Stages of Change Model.
Pre-contemplation: You already introduced this in Section I; however, in further detail discuss the reasons you spent time in this stage; barriers, obstacles, lack of comprehension, etc.
Contemplation: Analyze and summarize how your life will change. What will you sacrifice for executing the plan? How will you adjust your schedule to create time for your plan? What are possible problems and how will you address those before starting? This should be approximately one to two substantive paragraphs. Preparation: Explain how the literature review impacted the decision about the direction of your behavior change plan. What did you learn about the issue you want to change? What mistaken beliefs (if any) did you have before the research? Based on research, what did you learn specifically that might work and things to avoid? Draw conclusions about how the research will guide your attitude/mindset when preparing the plan. This should be at least four to five substantive paragraphs.
Action: Describe how you will execute your behavior change plan. Include environmental, social, emotional, and physical strategies and components. How will you address wanting to take action Include backup plans if there is a problem along the way. How will you keep track of progress? Who will assist you with motivation? This should be at least three to four substantive paragraphs.
Maintenance: Describe what happens when achieving your goal. What plan will ensure you do not backtrack and return to the bad behavior? This should be at least one (maybe two) substantive paragraph(s).
Transformation/Termination: Describe in detail the type of person you will be when you have successfully accomplished the goal. This should be at least one (maybe two) substantive paragraph(s).
